Checkbox doesn't appear in the form.

sorry for the delay in response, I haven’t had time to look at this plugin for a while. I can’t replicate your problem, checkboxes and acceptance fields display fine for me. Also the markup follows Bootstrap conventions, so there’s probably in your theme’s CSS that’s messing it up. To make sure it’s not a problem of the way you use the field, could you please show me how you’re including it?
