Ok that helps on width, but that doesn’t resolve the fact that according to that site “by default check boxes are rendered side by side.” This is not happening.

Looks like you’ve put both the checkbox items in separate CF7 checkbox tags for some reason.
If you put both options in a single CF7 checkbox tag as follows, you should see the default behaviour.
[checkbox your-rsvp "I am attending" "I am not attending"]
Following CSS in your theme (http://robandmarywedding.com/wp-content/themes/eternity/style.css?) is causing this:
