    I have solved this issue but thought it worth reporting here.
    I had upgraded the site to use https but the link in the configuration was still using http so anybody trying to sign up got a network error. Updating the configuration solved this issue. The documentation mentions that the error can occur with cross-domain linking or going from www. to no prefix, but not this. Unfortunately the author’s website is no more so the documentation cannot be updated.

    In order to see the Appearance > Widgets screen I had to install the Classic Widgets plugin.

    My site is at release 5.9
