I don’t mean to hijack this thread, but want notifications on it if they get back to you as I’m having the same issue.
It doesn’t seem isolated to any environment, but the plugin itself.

We just got it sorted. Turns out the client hadn’t paid a bill with eWay and once done and keys re-entered, all was fine. Hopefully it’s that easy for you!

Oh, very interesting!
It seems like if there’s any errors, the form just doesn’t display.
I just got ours sorted too. In our case it was 2 things:
1. The eWay API key password was too long (limit of 20 characters).
2. The customer country code is a required field, so I added the country field to checkout (we don’t use addresses at all in this case, but a country field isn’t too much to ask for this reason).
I hope all this helps someone else going in circles like I was.
