Categories not added to Stores when store is initially added

I am entering dealers (stores) in our site, http://www.bolandtechnology.com, and find that consistently, the category assigned does not save when I “Add Locations”. It always works when I “Manage Locations”, but of course, this makes entering each dealer a two-step process.

Any help would be greatly appreciated.
