It seems this form has been customized. Could you please confirm whether it has been modified or try creating a simple form on another page and integrating our payment fields?

No this form is not customised. The form has been working without any modification, it is only now that the inputs are not visible.
I have also performed the following:
- Replaced plugin with current from WP
- Disconnected and Reconnected Square
- Deactivated all plugins one by one
I have created a new simply donation form with no adjustments here – Testing Donation form – https://www.cancervoices.org.au/testing-donation-form/ the inputs are still not available.
Is there an email address i can send more information to, such as login details?
