I was having this issue and the plug in causing the issue for me was the Postible Facebook Content Management plug in. This is the 2nd Facebook type plug in that has caused a problem like this for me so I’d suggest you have a look at any you may have installed. Deactivating this mod was all I needed to do to get the select files button working again. The Facebook plug in didn’t even work 😉
Thanks for your advice. I think it has something to do with my customer’s server and not anything with the wp setup or plugins. I’ve deactivated the plugins and switched themes back and forth nothing worked.
They have a windows server and I think it’s some kind of permissions issue I just don’t know enough about servers to figure it out.

I have the same issue. the media upload window loads (slowly), I click the search image and nothing happens. within a post or in the media library.
It doesn’t work if you search for images either. search works but no thumbnail just title. then if you click on show nothing happens.
So effectively I am unable to add images to posts or to library
I am using wp3.3 on twenty eleven with w3tc and cloudflare. I have deactivated all plugins and same issue
Same. Only my problems is Events Manager, nothing to do with Facebook. The QuickPress on the dashboard works, but no where else, posts or pages. I have gone through all the suggested steps to no avail.
This resolved it for me as well. It was WP-TImeMachine on WP3.3.1
I am having the same issue. I have tested with a new wordpress installation with only nextgen gallery plugin. I have activated the default themes twenty ten/twenty eleven. On click ‘select files’ button nothing happens in firefox. It is working correctly in chrome. I have checked with all the previously said solutions but didn’t work at all.
There is a div with class=”plupload html5″ and opacity:0, which contains an input box with type=”file”. If we position this div in the same position of the button ‘plupload-browse-button’ ‘Select files’ will work in all browser.

I Deactivate all Plugin but it’s not working.
I got this error :
“logo.png” has failed to upload due to an error
The uploaded file could not be moved to /var/www/vhosts/xxxxxxxx/httpdocs/wp-content/uploads.
I check my permission and that is also fine.
I have done everything that anyone has suggested and the media insert/upload button remains completely dead – cannot load any media, doesn’t even want to try. Never had any problem before change single media button instead of 2, but I can’t say specifically when this happened because I’ve only just tried to upload new images. There’s a Spanish site that suggests its to do with http:// and that if you use https://all is OK. I tried that and it hung up completely, wouldn’t even load the site. Have tried IE9, Firefox, Seamonkey. Also Palemoon(based on Firefox) now crashes completely. Something ain’t right here….any ideas from WordPress cause I’m STUCK.
Same problem, deactivated all plugins, switched to 2011 theme, and still can’t upload images via the new tool or the old “browse” tool. The server is a windows server. I’ve been told permissions are ok.
My problem is fixed. I deactivated plugins and themes and nothing worked but when I cleared safari’s history, I was able to get it working.
