Can't see media library when using Form Lightbox plugin

    Hi, when using the Form Lightbox plugin I can’t see anything in the media library tab when uploading images. The images are also reported as failing to upload, although they appear in the appropriate folder in wp-content etc. The media library works fine when accessing it from the sidebar (Media>Library).

    Disabling the plugin solves the issue. Older versions of the plugin also cause this problem. The error still occurs when all other plugins are deactivated.

    I also see php errors after enabling debugging:

    Has anyone else experienced this problem, or know a solution?
