Can't save list settings – 404 error

I’ve extended the plugin to allow me to send newsletters to my Constant Contact list. When I select an email list and click on “Save Form List”, I’m redirected to a 404 page. Any idea what could be causing this? I think it has something to do with there being multiple lists, as on another website it works fine when there is one list.
P.S. I also bumped this thread on GitHub, but since there was no reply on the thread, I figured I’d post it here too.
