On both Apache and IIS installations (I’ve tried on 6 different servers now with one local). I can’t seem to find the offending line of code because it’s only trying to modify once. Has anyone else seen this?

  • Make sure that you don’t have any blank lines before the opening <?php and after the ending ?> of those files mentioned in the errors.

double check the wp-config.php file…. that’s been the biggest culprit as of late.
