First rule of jumping into a thread, try all of the troubleshoots steps already posted. 😉
If deactivating all plugins and trying Twenty Fourteen didn’t work for you, try downloading WordPress again and delete then replace your copies of everything except the wp-config.php file and the /wp-content/ directory with fresh copies from the download. This will effectively replace all of your core files without damaging your content and settings. Some uploaders tend to be unreliable when overwriting files, so don’t forget to delete the original files before replacing them.
I am sorry, but I am extremely reluctant to fool around with my highly customized Thesis theme, just to get the edit/delete prompts back on my images.
I, too, am having this problem and I would respectfully ask for someone to consider a fix or a better way to deal with it, than for me to have to spend half a day fiddling around with my internal files, potentially opening up the floodgates for real problems.
Evidently, by clicking the upper left corner where they used to be will, in fact, get you to the edit tool screen. They’ve just gone invisible.
That seems like a buggy sort of thing to me, and perhaps someone might take a look at repairing it? Thank you.

owlsdaughter, in a different thread, it was already mentioned that the developer of Thesis released a fix for his theme under WordPress 4.0. You should ask about that at http://diythemes.com/

I am using responsive theme and after update of wordpress 4.0, I am also facing the same problem neither I am able to access the media files/images nor getting access to any other plugins like shortcodes, YoP Polls, Meta Slider etc. Please help!
