Hi When trying to answer a question the user is redirected to a page “smart-form PDF” which says it is trying to load the document but this fails. should the user be able to answer the questions on the page

I activated twenty eleven theme and it worked ok but I did not want to use that theme.
I re checked the page and somehow some script was added to call up “smart-form PDF” I have deleted this and re inserted [vfb id=’1′] and now all is OK.
