Awesome, thanks! A couple of suggestions: First, CiviCRM has a DeDup function that allows you to avoid creating a duplicate contact. If you add a dropdown box to select the DeDup rule to apply, that would allow the user to control which fields will be matched to determine if there is a duplicate. At the moment, it looks like you are only matching to existing contacts on the email, but emails are not unique in CiviCRM so it won’t work properly.
Second, a nice-to-have feature would be to allow us to create an activity in CiviCRM when the contact is created. The activity can be assigned to someone who will handle the incoming contact.
