Do you have at least one payment gateway activated under Events > Payment Gateways?
Yes, only one active, Paypal.
Is this happening for all events or just one / some?
It’s possible this is a conflict caused by your theme or another plugin. To check if it is, can you test with all other plugins deactivated and while running the default WordPress theme?
I deactivated all plugins and set theme to twenty seventeen.
The booking form came back and activating them all in turn showed a problem with NextGEN Gallery by Photocrati Version 2.1.26, https://wordpress.org/plugins/nextgen-gallery/
However, after updating to Version 2.1.60, the booking form is back.
I found some issues with an Instagram plugin and a jQuery updater but I don’t think they are related to this issue, just a consequence of deactivating/reactivating.
