Have you copied the shortcode from the CostCalculator dashboard (Screenshot)? The Quotation marks in your shortcode is different and may be the root of the issue.

I do see that and yes I copied from your dashboard manually as well as manually added quotation marks. Please review the turf-test page again where I added multiple tries PLUS another shortcode for a form that displays correctly.

I could not recreate the issue in our testing environment and shortcodes working properly.
To check the problem in detail, please disable third-party plugins and activate the default theme of WordPress (for example, Twenty Twenty-three). This will help you to understand what exactly is causing the problem.
If problem persists, please share screenshots from the page.
