BuddyForm “Add New” page becomes blank

I installed BuddyForms and created a simple Contact Form which worked well. I then tried to create a second form by clicking “Add New” in the left side menu. The screen changes to a white page with the title “Add New Form”. A fraction of a second later the title disappears, leaving a blank screen. The other BuddyForms menu items work correctly but if I try to edit my original Contact Form the same thing occurs but with the title “Edit Form”. The back arrow on my browser (Firefox) returns me to my Admin page. My original form continues to work.
