I’m on a budget so looked hard for a free drag & drop forms plugin that can do multiple columns with labels on the left, and supports HTML fields, and this is the one. Much gratitude to the developer for a superior plugin. (I did have to add a little CSS to accomodate long labels, but being a developer, that’s very simple.)

ETA: after writing the review, I discovered that the plugin also handles conditional logic flawlessly.

You’re very welcome. I also wrote too soon, it turns out I needed some conditional logic, and was amazed that your plugin handles this beautifully.
