Break AVADA theme

I have Avada Theme installed. We have been using the older version of the plugin successfully for a while however when I upload the latest version of this plugin it broke the tabs functionality in Avada. It’s definitely the new plugin as I have it here http://iaocr.com/my-test-page/ and as you can see this site is running the older version and the tabs work fine. However if you try and submit the form http://iaocr.com/my-test-page you will see I get an error message, no download link and nothing appears in my inbox. I have delivery set to both.
