Booking Not Created Error

I have a test event at https://michiganfirst.com/events/test-event/ that is giving me the error: booking not created with no other info. Even if I don’t enter in any CC info, or bad info, I get the same error. I’m guessing the problem is in the form itself instead of authorize.net but haven’t had any luck figuring it out.
