edit comments.php of your theme and try to locate the line with comment_form()
to chnage it.
however, I don’t see this output below the comment form in Bold-Life;
(just installed on my test site)

Still in the process of setting up the site – but, at this stage, comments are on all pages – so the problem is on all pages. Website is segmentationstudyguide.com
I see not even any comment form, in your site;
because of:

as you don’t seem to have version of the theme from the download at http://wordpress.org/extend/themes/bold-life (despite the same link in your style.css) you might need to post comments.php (see forum rules how to use the pastebin) if you cannot successfully implement the sugggestion from my first reply.
neither the WordPress.org download and nor the WordPress.com version have the comments ‘html’ part.
exactly as described in the linked topic in my first reply:
try and change this line (4 lines from the end of your code):
<?php comment_form(); ?>
to:
<?php comment_form(array('comment_notes_after' => '')); ?>

no need to re-install the theme if you are otherwise happy with it.
