aweber footer slide up isn't doing what I need…

My blog is at: http://thedigitalcycle.com I’d like to remove the logo from the left side of the footer slide up and add the name box, email box and subscribe buttons to the slide up. I’ve been trying but with no success. Can you help.. Thanks
