Auto populate city & state based on zip code

    Is there a way, method, extension to get Form Maker to auto populate the city and state fields based on the user’s inputted zip code? If so, can someone assist me in learning how to accomplish this? Any help would be greatly appreciated. I have seen this on other form, but I cannot find any posts on how this is done or to learn how to do this.
