Hi, did you update one of your other plugins or theme? Do you have a cache plugin running in your site? If you do, did you check to make sure your cache plugin is not causing this issue?

Yes, many plugins have been updated.
My cache plugin shouldn’t be causing this. It wasn’t updated recently and I have had it installed since before we started using Google Authenticator. They have worked together well for over a year. I have cleared the cache regularly since the issue began, but to no avail.
Are there any known issues I should be looking out for?

I read another post where you recommended a different plugin, and I ended up switching so I can just do email-based two-factor authentication. My users have had some confusion with setting up Google Authenticator, and when they switch phones, it’s a nightmare. Thank you for your help, and I really liked your plugin while I used it!!
