Same, here. Switched back to 2.5.10.2 and PHP 7.4 and set the File Path to Media Library. Now it works. Need to test it in a staging enviroment.
We have a similar error. Sometimes attachments are sent (with smtp), sometimes they a stored secure (without notification) and sometimes just put in the media library.
The plugin should have a function, that if sending the application fails or saving in secure location fails, the applicant should get message “An error occured, please send in you application via E-Mail”
Same issue.
Changing the save location to WPMedia Library is not working either.
No more attachments send.
Any solution to fix this ?

There is an update, but sending mail fails with cv. works only with 1 pdf.
Everything is stored in backend but sending a mail never finished.
On 3 Sites i have to use this solution.
Switched back to 2.5.10.2 and PHP 7.4 and set the File Path to Media Library.

I hope Bluglass guys give us a reply concerning this bug. There is also a security issue concerning the latest version of the plugin.
I am having the same problem. The files are there but they do not attach and send with the email
Same problem for me :
- attachments are anymore sent as usual by email,
- they are not uploaded on the website’s WP Media Library when choosing the “WP Media” plugin’s option for the files storage
- and they are not uploaded on my site’s server when choosing “Secure Location” …
So I switched back to 2.5.10.2 and set the File Path to Media Library, and it worked fine. But this is not a good solution as I won’t allow me to get new versions …
I’ve sent an email to info@blueglass.ee. Waiting for an answer …

@hznknd works fine on PHP 8.1, fix was implemented and tested on host running 8.1 🙂
