Attachment FAILS! process doesn’t get completed

    I just tried attaching an image.

    After I select a file to upload, the progress bar stops to full, but it doesn’t show a green tick or any confirmation that the file is successfully attached.
    it seems the upload is not working or stuck.

    further if I delete the attachment, it shows error (may be because the attachment doesnt get completed). and I have to click the delete icon again.

    Upon inspect element, the console shows the following error:
