We ended up using a paid solution. Don’t have it present. But install the plugin on a new site and you’ll see. Others have also experienced the same issue.

Yesterday, we released an update to the plugin that fixes the issue where the state doesn’t populate when the store is set to only sell to one country. Give it a try and let me know if it works for you now.
