What content do you have in the Form tab panel?

For what reason do you put <br /> and <span></span> in unnecessary places?

I’m not really sure, I had copied that code from somewhere like 7 years ago. It had worked fine until just recently.

Thanks for pointing that out. I took them out and the alignment issue is fixed.
I don’t have the extra <br /> and <span></span> in my formulas, but all my forms are misaligned after the update as well. The of the pages can be seen here:
