Ajax domain checker with woocommerce some field

Hi
i have form field

username :
email :
phone :
url :
themes : (drop down)
[Submit Button]

and if user press on submit it goes to call the script to process some function and store to mysql.

i would like to integrate with woocommerce so customer can choose their domain and to pay first either by bank transfer (manual approved) or by paypal, if the payment not yet approved/received this information will not execute until payment/order status are successfully. is your plugin are able to do this? thanks
