I upgraded to wordpress 3.5.1 and now my contact for does not show on the website. Please help as we get a lot of business through this portal and currently it is showing the following

I am having the same issue.
I am able to use the contact form, but the form stopped redirecting.

I found, I finally DID find what was wrong !!!
Not only, you need to have the <?php wp_footer(); ?> called in your footer but also make sure the <?php wp_head(); ?> is called in your header !
And this, no one ever mentioned it on all the forums I’ve been looking for !
