After 4.0.1 Upgrade Not displaying – upprev_box DIV not in page

Hello,

Been using this plugin with good luck. Just upgraded http://www.thedieseldriver.com to 4.0.1 and, while I do see all the other page source items for upprev, it doesn’t seem to be building the actual content div (#upprev_box) and, thus, doesn’t display.

I checked the settings and compared with http://www.frequentbusinesstraveler.com, which is on 3.9.3 and working fine.

Thoughts?
