 * Looks like the list associated with your form is no longer found in your Constant
   Contact account. To help try and resolve that issue, can you take these steps:
 * * edit the form in question in your WP admin to not have an associated list
    *
   save the form * Visit “lists” page, and click the “sync” button at the top * 
   Re-edit the same form and re-select the list you want associated
 * Hopefully those steps resolve the issue and get you back to collecting signups.
