what version of PHP are you using?
These are warnings thrown by newer versions of PHP and are not errors per say. The first one is related to this plugin, but the last 3 are related to WordPress, I too get them on my server.
The warning in the plugin will not affect its functioning. I will remove the warning in the next release.
Hi I’m unsure if this a fix as I have the same issues but not just in admin.
the form is still working but on redirect you just get a white page with the following.

PHP 7.3 or 7.4 no change – everything was ok until I updated to 3.3.3 earlier today. at one point I had to deactivate before I could do anything.

Also get this after activation.
The plugin generated 259 characters of unexpected output during activation. If you notice “ headers already sent ” messages, problems with syndication feeds or other issues, try deactivating or removing this plugin.

@maxms I realise my first answer to your post was incorrect, apologies. I don’t yet understand why this warning is being raised as that line has not changed in v3.3.3. Nevertheless, it is now redundant and so I have simply removed it in v3.3.4. Please upgrade and let me know if it now works for you.

Thank you for the quick update – that has fixed it and now it works as before.
Regarding redirection, a very good question I have a tab that will redirect to a page within my wordpress site. looking at my installed plugins I have this Plugin and it has worked alongside yours without any issues until today.
