 * How do I add more fields to the donation form in addition to the First & Last
   Names / Email address? I am creating a website for someone running for State 
   Assembly and he needs the address & location of donors so he can follow up with
   physical mail.

 * For custom fields you have generally two options:
 * 1) You can create custom fields with code using [our documentation](https://givewp.com/documentation/developers/how-to-create-custom-form-fields/).
   And here’s [a working plugin](https://wordpress.org/support/topic/adding-fields-address-to-donation-form/github.com/WordImpress/give-snippet-library/blob/master/form-customizations/custom-fields-plugin.php?output_format=md)
   based on that documentation.
 * 2) You can purchase our [Form Field Manager](https://givewp.com/addons/form-field-manager/).
   That will allow you to add as many fields as you like to your forms, though we
   always encourage folks that the fewer fields the better to optimize your donor
   experience.
