it seems, your problem is not related to my plugin. You need to test your environment by deactivating all other plugins, clearing server and browser cache.
If you give me an URL, I can check it, but I think, this is an issue with your other plugins or custom configurations, settings.

I had already cleared all caches (Browser + server)… Would definitely appreciate you checking it. The site/all other plugins/functionality were working fine on the site prior. I just added Gravity/Surbma under 2 hours ago. All existing plugins are essential

Thank you for the URL! This is nothing to do with my plugin. The Gravity Forms own CSS is not enabled. You need to enable and use GF styles. It can be enabled on the Forms -> Settings page.
Thank you, I tried that and still no luck :/ As soon as I change a font size in Divi Builder all the formatting (the addition of bullets and smaller width of fields) glitches.

I’m sorry, but I can not give you support for debugging your site. The Gravity Forms css must be enabled to display the fields correctly. Any other issues you have, you need to find a developer to debug and fix it.
