I’m trying to do the same thing. I can’t find info anywhere on adding fields. Have you come any closer in the three months since you posted the question here?
Country list is comming up in 1.15.0, maybe earlier in 1.14.0 🙂

Download the 1.14.0 RC1 on the mainpage and you can adding your own selectbox in this version. Be careful 1.14.0 is only a release candidate and only suitable for testing.

Dani, are you planning to allow more user fields than just the ‘additional selectbox’ capability provided by 1.14?
I’d like to be able to add 2 additional fields.
Can this be done by modifing the post/form default.tpl files?

There isn’t a plan to do that, maybe in a following version.
You can implement this by your own but it’s not enough to edit the post/form/default.tpl file, dmsguestbook.php has to be changed and the sql database also.
