    I added the require statement for it to the end of the section with the other extension require statements in the achievements.php file.

    Don’t edit the plugin’s files! Make your own mini-plugin to extend Achievements, otherwise you’ll loose your changes when an update comes out. Look at what I suggested in the first link you quote in your post.

    Otherwise, I’d suggest you try making a helper plugin in mu-plugins, and try something really trivial. Hook a function to one of the actions you’re trying to capture, and put some debug in the function, and then do the thing that triggers the action. See if your debug code is executed (i.e. stick a die() in with a custom message).
