I need to insert new fields on the form, how I can proceed?

I also have the same exact question. I cannot figure it out.

I can’t believe nobody has an answer for this. anybody know?

I have same situation here,

i want to implement an user input (Dynamic input to form) Click here to send email to : (when the radio option clicked) the jquery handle a add Input, same as this http://sroucheray.org/blog/jquery-dynamic-form/ but i dont know how to implement it.

Anybody here could help us.?

You must go to smcf.php and look for function footer(), and there you can edit easily the HTML.
