I am having the same issue.

I am having the same issue. Nothing is happening
http://corklike.com/wp-admin/post.php?post=4044&action=edit#
I have deactivated plugins individually and there is still an error.
I too having the similar problem. After clicking the “Add Media” button the pop window does not shows. Please help!
I have the same issue with the “Add Media” button. Have not tried “Insert/Edit Link”. I have had no choice but to revert my site back to WP version 3.9.1 whilst the issue is resolved.
I am using the hosted version of WP on GoDaddy so I don’t think I have that option. I am new to using WP. How quickly are releases pushed out to address what seems like a bug in this case?
Same Problem here! Any news on fixes? Anyone?
Hey Ridgeback21 how do you revert the site back to WP version 3.9.1.
I need to be able to add the photos in posts.
My Webhost provider allows me to keep back up files of the WP data whenever I wish. Now that I am back to WP version 3.9.1 everything is working again. So I will update each plug in and update, one at a time. Then, if all ok, will attempt to upload WP version 4.0 once more, whilst keeping back up files at each stage.
I’m having the same issue. When clicking the Add Media button nothing happens. I cannot add any photos to my posts. I also have the hosted version of WP on Go Daddy. I have not been able to do anything to correct this.
Same issue here. I was able to use the add media button until yesterday! Please fix this!!

@5star7day, have you tried the general 4.0 troubleshooting steps? If so, did you get stuck? If not, see: http://wordpress.org/support/topic/wordpress-40-master-list
Everyone else, if you’re not familiar with WordPress.org forums; These are support forums ran by volunteers. Demanding that we fix your issue won’t encourage us to help you.
If you are absolutely certain that your issue is the same, and you’re not just seeing the same symptom, then at least for common courtesy create your own thread to discuss your own issues: http://wordpress.org/support/forum/how-to-and-troubleshooting#postform
I got the solution. The Comprehensive Google Map Plugin was breaking the path to media files. After deactivating it all is well now.
Thanks! This worked for me. Seems to be working perfected after deactivating the Comprehensive Google Map Plugin.
If a plugin is using Google Maps could this be the error. I am using The Events calender pro plugin (which i did test deactivate but I am still seeing the error.
