 * i would like to add new fields to my form but it is empty, it does not offer 
   anything…
 * I saw on screenshot that i should be able to choose amoung elements but mine 
   is blank…
 * Any idea ?

 * Looks like there is some issue with ajax calls in your site.
    If you could provide

 * Thanks for your reply ! Finally i had a big problem with my wp blog, i had to
   delete all and re-install the totality. Now i use another contact form, so my
   topic is closed…
