Add attachment when submitting

Hi @kenrogers we don’t support upload/photo fields in our customer information fields unfortunately. If you’re looking for a way to do this, our Plus edition (this is a paid option) does have a Gravity Forms integration that would let you build a full-featured form with appointment scheduling as part of it. This is a detailed example of how you could use the two plugins together:
