Add amount if checkbox is selected

    WooCommerce Checkout Manager 4.2.3
    WooCommerce 3.4.4
    Wordpress 4.9.8
    URL / URI not available – it is in coming soon mode

    In the Additional settings I added two separate checkboxes for extra shipping options (upper floors delivery, telephone notice); when checked, each of them should add a different extra fee on the total.
    I tried to play a bit with “Add amount” and options but I could not manage to make the extra amounts conditional: they were fixed, or they did not show at all, regardless if the checkboxes were ticked or not.

    Any help? I am also open to change input type, if needed.
