Add a specific checkbox field

Hello,

I would like to know if there is a way to add a checkbox field inside the reservation process.

I need users to add an information if they are members or not (just check or not the box) somewhere during the reservation process and this info is visible in the mail sent to the admin and in the reservation calendar.
