Add a class (under Display Tab)

I wanted to see what was involved in adding a class to the form. I know this can be done through the filters but it involves identifying the form by key, which is not convenient in most cases.

I ended up prototyping the functionality directly in my copy of the plugin, and it was a simple change. I’d be happy to share my modifications if there’s interest
