How do I recreate a form

I created a site quite some time ago and I’m now in the process of updating. Problem is, I’ve forgotten virtually everything about how I built it. So, here’s the question. I created a page for a request form (see scan below)

Pretty simple, right? Now, all I want to do is recreate this layout/style as is, and repurpose it. If this was a web page, I’d simply copy/paste/rename then edit the code. But that does seem to be an option, and I can’t find the page on the website. I’ve tried sooo many times to recreate this from scratch and I’m losing too much hair.

Any help very, very much appreciated.
