    Hi. Thanks for your plugin. I’ve been able to get it to do most of what I wanted, which is great.

    I have a few bugs to report.
    1) I can confirm the same problem as the other people reported with the form displaying in the top left hand corner of the Content area.
    Editing the index.php file as suggested by kcsck in this post works well. So please make sure this code is added to your next release of this plugin.

    2) Your shortcode has a typo in it. “Ulitmate” instead of “Ultimate”. When you fix this, please make sure your plugin still recognizes the incorrect spelling, so that people’s current shortcodes keep working.

    I will log suggested improvements in another support request.

    Just remembered one more little thing to fix – there is a
    after the promo code label that shouldn’t be there. There’s none after the email label (which is good), so just need to delete the one after the promo label.
