• In my setup, (WP 4.3.1 on IIS 7) I had to define a dummy domain prior to defining the domain I wanted.

My main site was, say, foo.mydomain.gr. To define a second site, say, moo.abc.mydomain.gr on the same WP installation, I had to define the domain moo.foo.mydomain.gr first, do the mapping and then change the name to moo.abc.mydomain.gr. This happened because the form adds foo.mydomain.gr as suffix to the new domain name.

This is not documented and I had a trial-and-error situation until I managed to do what I wanted.
