Support submit form fails

I have several questions but the support form inside wordpress keeps failing so here they are:

I created the guest post pages as shown in the video. Everything seems to work except that there is no toolbar showing up on the entry form, only the Visual and HTML tabs. No buttons at all.

Where do I find the name, email, address, and private notes info collected in the form? What if the person does not have a website? It gives an error if left blank.

I also responded to your last email on the Amazon link redirect problem. Let me know if you need further info on that issue.
