user avatar plugin not working

I have the latest WordPress and the latest version of user avatar plugin. I have no other plugins installed and I’m running on my own dedicated server with latest php etc

When I upload an image to my user profile it works and I can see it on the backend but when you use php or short code to hook into the front end of the theme it does the generic mystery man gravatar.

The correct custom avatar also shows up in the admin toolbar

I searched these forums all day for a solution and couldn’t find one.
I’d appreciate the help.
