Age Gate plugin?

Since age gate is required when uploading any game videos with gore or the like (even though it displays a rating before the video starts), I can’t upload them all to Youtube (not a partner), therefore I’m looking for a plugin which would allow me to put age gates for videos that I upload on my site.

Is there an age gate plugin around? I couldn’t find one…
