[Plugin: Fast Secure Contact Form] Having an issue with this plugin & a URL

I have created a page on my site for nightclubs & really anyone to submit their events. I created the form just how I want it, however when I go to the URL http://thegoodlife-miami.com/submit-your-event/ it’s missing a few of the “extra” fields I created. So now someones going to go through all the steps to submit their event & cluck submit, only to find out they’ll be taken to http://thegoodlife-miami.com/submit-your-event/#FSContact2 where it shows ALL the fields. I have no idea why in the world it’s doing that and I have NO idea how to correct it. I also setup a post “http://thegoodlife-miami.com/thank-you-for-submitting-your-event-html/” that you’re supposed to be taken to after you successfully submit your event, but it doesn’t do that either.

But my main concern is the first issue I touched on. Anyone that has some insight into this I would greatly appreciate it!
